Cancelation Policy:
I understand that a cancellation request form must be completed on the DHP Health & Fitness website.
I understand a 4-week [28-days] notice is required and any invoices scheduled to charge within that 4-week [28-days] window will be billed in full.
I understand that should I cancel before the agreed-upon time designated by my membership plan and rate, I will be responsible for payment of the difference in price for the plan I should have been enrolled in based on the actual duration of my membership.
